03 Jan Final Deadline for 2018 Desk Reviews – May 31, 2019
Create your 2018 desk review forms on-line by logging into the Users area and submit all forms by midnight Jan. 31, 2019 to avoid late fees. After all desk review forms are ready, complete the desk review checklist and hit SUBMIT to notify the IPMAP that the desk review is ready for audit. Please remember to save all forms as PDFs on your own computer or print and save in your IPM binder as they will be needed during your next on-site audit.
Once you have hit the SUBMIT button for the desk review checklist, staff at IPMAP at the University of Guelph, Ridgetown Campus will check your submission for completeness and notify you via e-mail that it has either been forwarded for audit or that something is missing.
Any desk reviews submitted after Jan. 31, 2019 are subject to the $500 + HST late submission charge and no 2018 desk reviews can be accepted after May 31, 2019 (the last day of the 2018-2019 registration year).
